Patient Reviews Report

Shows drug reviews for patients registered at your practice. Includes patients whose treatment is managed elsewhere.

Use this report to:

• Audit completed review questionnaire data for patients on non-warfarin anticoagulants and related drugs
• Track next review dates to plan upcoming clinical activity
• Review summary counts of patients and reviews, grouped by registered GP practice

The report covers four drug categories, each in its own section. Only completed reviews are included:

• Warfarin – Compliance, CHADS₂-VASc, HAS-BLED, ORBIT, 12 Month TTR, All Time TTR, number of treatments and average review period (last 12 months), next review date
• DOAC (Apixaban, Dabigatran, Edoxaban, Rivaroxaban) – Compliance, Dose, CHADS₂-VASc, HAS-BLED, ORBIT, Weight, Creatinine, CrCL, ALT, Haemoglobin, Initiation Review, Blood Test Required, next review date
• LMWH (Dalteparin, Enoxaparin) – Compliance, Dose, CHADS₂-VASc, HAS-BLED, ORBIT, Weight, Creatinine, CrCL, ALT, Haemoglobin, Anti-Xa, Platelets, Initiation Review, next review date
• Acenocoumarol – Compliance, Dose, CHADS₂-VASc, HAS-BLED, ORBIT, INR Result, Target INR, next review date

All sections also display Patient Name, NHS Number, Patient Number, Drug, Review Date and Diagnosis.

Each section contains a summary table grouped by registered GP practice (showing patient count and review count) and a detail table with one row per completed review.

Parameters

• From Date / To Date – date range filter for completed reviews
• Registered Practice – permission-filtered multi-select controlling which GP practices’ patients are included
• Drug Type – multi-select to choose which sections to run (Warfarin, DOAC, LMWH, Acenocoumarol). The report does not run until at least one drug type is chosen and View Report is clicked.
• Page Break Between Locations – Yes/No; when Yes, each registered practice starts on a new page in the detail tables
